But all is not lost. We’ve learned a lot over the past 30-40 years, and we’ve made progress in many areas. There are approaches and mental models that have led to certain levels of success. And today’s guest has been participating in and observing many of them over the past few decades. Today we talk with Alan Weil, Editor-in-Chief of Health Affairs, the nation’s leading health policy journal.  Alan takes us through the history of Medicaid Transformation from the early 1980s on. Sepsis is not an infection. Rather, it's your bodies overwhelming reaction to an infection and can lead to some serious health issues. In fact, sepsis is the #1 cause of death for patients in hospitals. Sepsis definitions and protocols have been around for some time, but have been undergoing major changes. Additionally, sepsis is under scrutiny from CMS at the federal level and is subject to public reporting in many states. All of this, plus the importance of quickly diagnosing and treating the condition puts clinicians in a difficult spot. Many times they'll find themselves treating to the protocol, even when their clinical instincts suggest something else. In that regard, sepsis proves to be a very instructive topic in our never-ending quest to unravel the business of healthcare. On this episode, we'll talk with Michael Ackerman who's an Acute Care Nurse Practitioner and an Associate Director and Professor at Niagara University School of Nursing. Michael is an expert on the topic who speaks on it across the country. He'll help us understand what we're up against and why it's so complicated from both a clinical and administrative standpoint. Sepsis is not an infection. Rather, it's your bodies overwhelming reaction to an infection and can lead to some serious health issues. In fact, sepsis is the #1 cause of death for patients in hospitals. Sepsis definitions and protocols have been around for some time, but have been undergoing major changes. Additionally, sepsis is under scrutiny from CMS at the federal level and is subject to public reporting in many states. All of this, plus the importance of quickly diagnosing and treating the condition puts clinicians in a difficult spot. Many times they'll find themselves treating to the protocol, even when their clinical instincts suggest something else. With richer data, CAQH can deliver increasingly useful insights, more precise measures of progress, and more accurate estimates of cost savings. The epidemic of physician suicide is costing us lives. Not only the lives of physicians themselves, but also patient lives. If we do not care for the caretakers, how many more lives will we lose to medical error? The cost of physician suicide has carried a heavy toll on me and especially my children. It is our hope that the system will improve, and that the physician community can have better support. What a patient does at home during the time between doctor visits has the largest effect on the outcome of their care. The Right Kind of Patient Education - Stacey Richter - QCHealth - 065

What happens when you take an experienced digital health buyer and an experienced digital health seller and have an honest conversation about the process of bringing new innovations into a health system? You get lots of not-so-obvious, but tremendously valuable insight into how to cross the moats successful health systems have been forced to build to keep you out. Co-host Shahid Shah recently published a post with Brian Van Winkle on The Healthcare Blog titled: Perspectives on Working with Healthcare Systems for Digital Start Up Companies | Part 1. Shahid, an experienced digital health innovator played the role of the seller. Brian Van Winkle, an expert on bringing new technology into hospitals, played the buyer. On this episode, Shahid and I discuss the key points from that interaction and tie them back to the principles we’re always talking about on The #HCBiz Show! We explore topics including:  How health systems make decisions How health systems contract What it means when they say health systems are consensus-driven and permission-based Why it’s critical to understand the budget process and procurement cycle How to determine who should be involved in the process and who has the necessary authority How volunteering can be a creative way to get on the inside Using BANT to guide you (Budget. Authority. Need. Timing.) How to balance the need to build a pipeline with the notoriously long sales cycles and the need to make sales now Using OKRs, or Objectives and Key Results to prove your value. Why there are 0 examples of software-only companies succeeding in healthcare. You’re a tech-enabled services company whether you like it or not. Getting to No! It can be the best thing for you and your prospect and it’s valuable to get there quickly. Looking at annual reports and SEC filings to find out what hospitals want to buy. (Jeff Byers talked about this on Episode 62 too) Why and how to get on the RFP invitation lists. And then why you probably shouldn’t reply to that RFP anyways! The magic of “Always Be Valuable (ABV)” Rule: If you don’t know your value then free is fine. If you know your value then free is a waste of time. Volunteering is for learning. Not selling. Corollary: Even if they give you $0, it’s still not free for them. They are spending staff time and opportunity time on you. Rule: If you’re asking them to do something for you before you’ve done something for them… stop! There’s always a value exchange. There are other things you can ask them for besides money (i.e. Present at an event together. A shared study. A press release. Etc.)    Selling into health systems is complicated, but it is possible to differentiate yourself from the flurry of innovators that are competing for their attention. With the proper perspective and a sound strategy, digital health startups can address the health systems unique needs and constraints to close deals. Robotic surgery is a loaded term. Just reading the words can bring Hollywood-inspired images of fully autonomous robot doctors to mind. But that's not what's happening in the industry today. The "robots" are merely tools used by doctors, just like a stethoscope or a scalpel. And they have some interesting use cases and value propositions to health systems. It gets specific about technology by calling out FHIR APIs. It gets specific about use cases, such as patient access to data, payer to payer interoperability for administrative data, etc. And surprisingly, it gets specific about collaboration by suggesting that all health plans look at the Da Vinci project and consider implementing its use cases. On this episode, Don Lee and Shahid Shah examine the CMS NPRM from the payer perspective. Our goal is to help you understand what opportunities exist within the rule, and we explore topics such as:  How health plans can benefit by making administrative data (i.e. claims) available to their members via 3rd-party app vendors. IPAC-03 Why Hand Hygiene Matters - Tamás Haidegger

In the rush to implement EHR and meaningful use, did we forget about interoperability? Congress thinks there should be more to show for the billions of dollars already spent on EHR. Now, they've asked the Office of the National Coordinator (ONC) to give guidance for the future. On this episode, Shahid and I sit down with Niam Yaraghi to discuss what TEFCA is and what it means for interoperability and the business of healthcare. The problem with all this is that no one can clearly articulate what interoperability (or data blocking for that matter) is. In reality, healthcare has already solved interoperability about as well as any other industry. If you show me two parties who want to exchange data, and are willing and able to fund the effort, then I'll show you interoperability. Further, there are robust regional Health Information Exchanges (HIE) throughout the U.S. And as I have said many times before, the regional HIE is the only near-term option to solving for interoperability in a way that resembles what everyone seems to be looking for. TEFCA, in many ways, assumes these things aren't already working. And because of that it may actually impede efforts that are already on the path to success.
