Caring for another person, especially a parent, can feel like navigating an unpredictable, wild river. How do we determine when it’s no longer safe for Mom or Dad to live alone? Moms Uncharted Path Into Dementia

Caring for a caregiving spouse is not a topic I’ve discussed on this show. Not directly. I was pleasantly surprised when my hubby told me he had an idea for an episode. To be fair, I don’t need someone to feed me ideas, but I listened. You’ll be glad I did because his idea was gold. My Mom died at the start of the pandemic, and life kind of went on the way it was unfolding back in March of 2020. We haven’t had a lot of conversations about our feelings, but that changes with this episode. Using Mindfulness to Reduce Frustrations

Conversations to Remember is a volunteer organization started by 16-year-old Aaron. Its mission is to connect high school and college students with residents in assisted living communities to combat the epidemic of loneliness. Putting a smile on the faces of seniors, including those experiencing cognitive decline, creates a bridge between seniors and youth. Aging Cause & Cure

There are many things a trustee needs to know… My sister and I were both a trustee of our parents’ estate. Because there are a lot of details neither of us was aware needed handling, it was a good thing for us. There are a number of complicated steps required by law when administering a trust &amp; that’s what we’re discussing on the show. You may be saying, “But, doesn’t having a trust avoid the legal work associated with a death?” Unfortunately not. When someone dies, the process is very nearly the same whether there is a trust or probate. The difference between trust administration and probate is that the trust administration is done privately, while probate is handled publicly in court, and usually, probate is much more expensive. In a probate, the executor is required to use specific form documents and to get a judge’s permission to act, but the basic steps that must be covered in probate are very similar to the steps covered in a trust administration. If you are the sole trustee as well as one of several beneficiaries, there are many steps you need to understand. One of the biggest challenges to caring for someone living with Alzheimer's or some other form of dementia is trying to figure out what's going on in their mind. They don't understand what's happening so they can't help us understand what they need. Not being able to communicate their needs causes fear or anxiety. Hopefully, today's conversation with Laurie will help us understand a bit better.  Laurie Gunter Mantz is the founder and CEO of Dementia Training for Life. She is also an Occupational Therapist, Educator and Certified Dementia Care Practitioner, Trainer, and Care Manager.  Dealing with both grandmothers in their challenges with dementia gives Laurie a personal insight into what we're all dealing with. Our conversation went to many places and despite some technical challenges, I feel this conversation is an important one for my listeners to hear. Interviewing Laurie before I launched Fading Memories would have been fantastic for all of us. That's how important our conversation was. Some of the things we discussed include identifying behaviors and determining why they are happening.  Laurie strongly encourages anyone who is showing signs of cognitive impairment to get a full diagnosis. In our conversation, she tells the story of a man who was misdiagnosed and how that impacted his life. We discussed what to do as our loved ones progress with their disease and much more. When you love someone with dementia it's not uncommon to feel like you're losing your mind too. In this episode, I talk to Beth Friesen about practical tips for coping with this problem. Beth is the author of the book “I Love Someone With Dementia, So Why Am I Losing My Mind?”. She's also a nurse in a family of medical professionals. If their family struggled, we shouldn't feel bad about our struggles. Some families have a member who doesn't fully understand the disease which leads to negative situations. Understanding how to be in “their reality” is a key survival tip for anyone who is caregiving. In fact, it took two years of explaining strange behaviors before it dawned on Beth that her Mother had Alzheimer's. Learning practical tips will help caregivers feel less like they're losing their minds and more in control. One of the first fundamental things to understand is dementia is more than memory loss. I like to describe it as an old computer that fires up but really doesn't do the computing we need it to do. In fact, it took two years of explaining strange behaviors before it dawned on Beth that her Mother had Alzheimer's. Learning practical tips will help caregivers feel less like they're losing their minds and more in control. One of the first fundamental things to understand is dementia is more than memory loss. I like to describe it as an old computer that fires up but really doesn't do the computing we need it to do. Two Lap Books – A Great Way To Connect! Reading the book together gave us the opportunity to laugh, talk about likes and dislikes, and even sing a little.  As a result, our visit was one of the better ones in a long time. By sharing the books with two other residents made them as happy as it did Mom. All three ladies enjoyed the beautiful illustrations and relatable scenarios. Because each resident has their own level of reading ability they were able to read on their own. Regardless, reading together gave all three got a tremendous amount of pleasure from the books. The Perfect Activity to do Today – Two Lap Books

Lewy-Body dementia is very different than Alzheimer's. While physical changes are similar, cognitive changes can be vastly different. Equally challenging, Lewy-Body dementia can present with a range of symptoms. These include problems with thinking, memory, moving, sleep and/or changes in behavior. To better understand the unique challenges of LBD I spoke to Nance Tobar. Nance is caring for her father who has LBD. A misdiagnosis caused severe changes in his cognitive abilities due to the unique pathology of Lewy-Body. Nance's dad, Joe, fell and became hospitalized. As a result, his medications were changed. Notably, they took him off of one medication that was stabilizing his memory. Consequently, the family lost 2-3 good years with their Dad. Early and accurate diagnosis is important because LBD patients may react to certain medications differently than other memory patients. A variety of drugs can worsen LBD symptoms. Joe was prescribed an anti-psychotic. Thankfully, Nance was warned of the danger before she gave him any. Anti-psychotics can be fatal to a person with LBD. About Lewy-Body Notably, LBD is the second most common form of dementia.  Generally, LBD can have three common presentations; movement issues, cognitive impairment or neuropsychiatric symptoms, which can include hallucinations. Joe had hallucinations. Regardless of the initial symptom, over time all three presentations will develop very similar cognitive, physical, sleep and behavioral features. The most common symptoms of LBD include:  	Impaired thinking, such as loss of executive function (planning, processing information), memory, or the ability to understand visual information.  	Fluctuations in attention or alertness;  	Problems with movement such as stiffness, slowness, and difficulty walking  	Visual hallucinations  	Sleep disorders, like acting out one’s dreams while asleep  	Depression, apathy, anxiety, agitation, delusions or paranoia  	Changes in body functions, such as blood pressure control, temperature regulation, and bladder and bowel function. Understanding Lewy-Body Dementia

Caregiver confessions is something we all have but don't usually want to discuss.  Wouldn’t it be nice to be a caregiver who never gets frustrated and is an eternal optimist?  If that describes you, I applaud your emotional strength and resilience. However, many of us providing care for a cognitively impaired loved one, do not fall into that category; I know I don’t. Similarly, most caregivers struggle with negative emotions, frustrations, and the uncertainty that comes from caring for someone with memory loss.  Helping people navigate this treacherous journey was the reason I started my podcast. For instance, wanting to help ease the burden of having negative emotions sent me on a quest to find the right person to talk with.  Subsequently, I found Kate, the host of the podcast, Ignorance Was Bliss. For example, she started her podcast because she had things she wanted to say and the conversations she wanted to have. Kate is also a trained psychologist and a person living with serious health issues. Naturally, she was an ideal person to talk to. Difficult Caregiver Thoughts (AKA Caregiver Confessions)  	I'm sick of having no life of my own.  	Dad tries to control everything I do—it’s exhausting.  	How much longer can I continue caregiving? It seems like there is no end in sight.  	My loved one has no clue what I give up to care for them. They think this is a normal routine.  	Everybody constantly wants a piece of me, and there’s nothing left for myself.  	Nothing I do ever pleases them.  	Mom is suffering so much and has no quality of life. Why can’t she just let go and die? As a result of some of my negative feelings about Mom, I wanted to find ways to see the positive in our interactions. Searching for ways to turn that negative narrative in my head around has been something I’ve worked on for many months.  Consequently, my conversation with Kate gave me some terrific insight that I'm sure will help you. Meanwhile, check out the other part of our conversation on Ignorance Was Bliss.  As a result of our 2 plus hour conversation both of us got exactly what we wanted for our individual podcasts. Caregiver Confessions (with IWB Podcast)

One of the biggest caregiver challenges is managing difficult behaviors. People living with memory loss don't realize they're being unreasonable. Difficult behavior is an unfortunate by-product of their disease. Navigating our loved one's behaviors and our own emotions is crucial to caregiving success. However, no one has a clear path to making that happen. This episode of Fading Memories dives into a few behaviors and how we might handle them.

However, there are many days knowledge just doesn't help end the frustration. Listening to someone living with dementia and what it's like is a window into understanding. It's the closest we can get to complete understanding of what it's like to live with memory loss. That's why my conversation with Janice Swink, a woman living with mixed dementias was so powerful. Janice described symptoms I'd not heard of and detailed some I had. I was unaware of the possibility of scent hallucinations until Janice told me about hers. I was aware of visual hallucinations, something she also has to live with. In addition to the challenges that come with living with dementia Janice details some of the social stigmas, she has to put up with. People who tell her she doesn't look sick (she refrains from telling them they don't look stupid). Using her walker gets her understanding and patience but the electric cart does not. One challenge Janice described was the inability to remember that she was having symptoms of a urinary tract infection. This caused an unnecessary delay in being treated and another day or two and she would have been hospitalized. If you listened to last week's episode on my state advocacy day, this is the challenge the state of California is trying to address. A Dementia Warrior Janice is a warm and funny woman and our conversation was terrific. She makes videos for Facebook and Twitter.  She posts about what it's like to live with dementia, dancing videos, and even what bad days are like. Living with FTD W/Janice Swink – Part 1

Research has linked social isolation to higher risks of physical &amp; mental conditions; Alzheimer's &amp; death are just two of them.  Becoming isolated frequently happens “naturally” and there are many ways to combat this issue. Discussing senior isolation came naturally with my guest, Rebecca Graulich. She's a past guest who started a foundation to cope with senior issues. Recording this episode of the podcast was originally intended to be an update but turned in to much more. During our conversation, we discussed ways to prevent senior isolation. Some of them are easier than others. Regardless, we need to do what we can. The first prevention is something we all need, a sense of purpose. Having a sense of purpose makes us less likely to succumb to the negative effects of social isolation. Hobbies are an easy way to have a purpose in our senior years. Volunteering is another method of preventing isolation by giving back. Having access to transportation is also important. If driving is no longer an option there are many options in addition to relying on family. Public transportation is one option but it's not always convenient.  Many communities have volunteer organizations that can drive our loved ones where they need to go. It may take some creativity to solve this issue but it's worth the time it takes. Caring about someone or something else will always give us purpose. There are many things to keep in mind when considering a pet.   If a pet isn't practical, perhaps volunteering at a shelter or helping a neighbor with their pets is an option. Pet therapy can actually lower blood pressure and stress. Caring for a pet is fun and rewarding for anyone who needs companionship. More Ways to Avoid the Isolation Danger Zone! Avoid the Isolation Danger Zone!

Visiting Mom, trying to connect and give her pleasant times is a bigger challenge than I ever expected. Visiting With Mom (Jen’s Journey)

We've all had the experience of not remembering. As we age some of us dismiss this as part of aging and some of us fear it's the first sign of Alzheimer's. How do we know who is right? Experts say that mild memory loss is normal as we age. Occasional forgetfulness does not mean you're developing Alzheimer's. If we're concerned about developing the disease, have normal memory lapses we easily focus on what we're forgetting.  Knowing the difference between normal memory issues and memory loss we should worry about is tricky. Our brains are capable of creating new brain cells at any age so significant memory loss is not normal. Brains are like muscles and need to be kept strong just like the rest of our bodies.  Lifestyle can have a huge impact on the health of our brains. What we eat, and drink, how much exercise we get, how well we sleep are all important factors into brain health. The other is to regularly challenge our brains by learning something new. Anything that forces you to “think hard” or forces you to really concentrate are good for your brain. Normal Forgetfulness VS Dementia Occasional lapses in memory are normal, not a warning sign of Alzheimer's or dementia. If you find yourself with some of the following memory issues you'll want to get checked out immediately.  	Forgetting names of people you just met or calling one person by another name?  	Do you occasionally forget where you left things like glasses or car keys?  	Occasionally forgetting an appointment or walking into a room and not remembering why?  	Are you easily distracted or have trouble remembering what you just read or the details of a conversation?  	More frequently being unable to retrieve information that's on “the tip of your tongue” It's time to consult a doctor when memory lapses become frequent enough or sufficiently noticeable to concern you or a family member. This is not the time to pretend all is well.  Early intervention may allow you to have more good years than not so call the doctor and have a FULL workup scheduled. The traditional pen and pencil memory test which usually takes less than 10 minutes may not be sufficient to detect a problem. Reversible Causes of Memory Loss Memory problems don't automatically mean you have dementia. There are many reasons why you may be having cognitive problems. Some reversible causes include stress, depression, poor sleep or even vitamin deficiencies.  Some prescription medications may also have side effects that cause memory issues. We've all had the experience of not remembering. As we age some of us dismiss this as part of aging and some of us fear it's the first sign of Alzheimer's. How do we know who is right? Experts say that mild memory loss is normal as we age. Occasional forgetfulness does not mean you're developing Alzheimer's. If we're concerned about developing the disease, have normal memory lapses we easily focus on what we're forgetting.  Knowing the difference between normal memory issues and memory loss we should worry about is tricky. Our brains are capable of creating new brain cells at any age so significant memory loss is not normal. Brains are like muscles and need to be kept strong just like the rest of our bodies.  Lifestyle can have a huge impact on the health of our brains. What we eat, and drink, how much exercise we get, how well we sleep are all important factors into brain health. The other is to regularly challenge our brains by learning something new. A common caregiver struggle is the lack of support from family members.  Author Irene Olsen and I discussed potential solutions during our conversation on this episode.  Irene survived to be an Alzheimer’s caregiver…twice.  Her father, to whom her novel is dedicated, was the first family member. Then, Irenes' sister-in-law, diagnosed with mixed dementia a month after her father’s death, became the second. Why An Alzheimer's Novel? Feeling her experiences as a caregiver were meant to go further than the confines of her own heart and using her background as a care group facilitator and a state Ombudsman Irene created a supportive novel Requiem for the Status Quo. Irene's goal in writing Requiem was to encourage and educate those called to take on the role of the family caregiver.  Irene's brother was an extraordinary caregiver for his wife. Irene was the go-to person for advice, direction, and the occasional caregiving day. Having been front and center on her father’s three -year Alzheimer’s path gave Irene an “edge” on experience. Family caregivers are often ruthlessly challenged by uninvolved family members who are quick to condemn, but reluctant to offer assistance. Caregiver struggles can be devastating when their decisions are challenged or questioned.  Family members not actively participating in caregiving sometimes give advice as a way to “help”. Fearing getting “sucked in” is a common reason others don't help more when caregiving is needed. Find out what each person feels they can help out with, giving them an active role in caregiving.  Physically caring for an aging person is not something most of us are “hard-wired” to handle. Thus, allowing families who can't participate in hands-on care to tackle other tasks is a huge benefit to all.  Involving Reluctant Family Members Involving family in caregiving can be quite a challenge. It's an issue that's been discussed before on my podcast but worth revisiting regularly. Many family members, feeling they are not cut out to handle day-to-day care, opt-out of helping altogether. Lacking nearby proximity is also a common excuse. So, how can we help them help us? First, ask them what tasks they can comfortably tackle.. Are they good at making phone calls to doctors, specialists, etc?  Can they handle deciphering insurance paperwork?  Maybe they can handle things like yard work or managing paid care providers. There are more tasks to be done than 1 or 2 people can handle. Finding one that each person is best suited will go a long way in maintaining caregiver health. Being the “lone caregiving more solid” is putting yourself on a dangerous path. Assuming the responsibility and full burden of care, many people march forward disregarding their own health.  Eventually, this badly wounded soldier is no good to anyone.  In reality, it takes an army to effectively manage the care of an older adult.  Failing to let others help with the practical burdens, results in a health and wellness declines. Not anticipating how overwhelming tasks will be handled is a major source of caregiver stress. Having others who can help anticipate and find solutions to things like incontinence, heavy lifting and wandering are important.  Both urinary and fecal incontinence are among the leading causes of nursing-home placement. A small or frail wife of a big man who needs help is another tough scenario. Tips for Caregivers Who Lack Family Support Lacking proper support, caregivers have more difficulty ensuring their loved one receives the care they need. Regardless of why there are ways to help yourself in your caregiving. First, find out what resources are available.  Before I started this podcast I was unaware of how much support is available.  Connect with your local Area Agency on Aging. They can familiarize you with the available options.  Also, check the many options listed on my resources page.  Start with the Alzheimer's Association. They are a wealth of knowledge.

No one decides to become an Alzheimer's caregiver. Alzheimer’s disease creeps slowly into a person’s life but never retreats. Eventually, the individual’s memory fails in too many important areas, and someone must keep watch. Round-the-clock care is needed, and that invariably falls to the ones who care most—one’s partner or children. Becoming a caregiver to your partner of many decades is difficult. Speaking to the former governor of Wisconsin Marty Schreiber helped me be a better caregiver. Marty has seen his beloved wife, Elaine, gradually transform from the woman who had gracefully entertained in the Executive Residence to one who sometimes no longer recognizes him as her husband. Candidly counseling those taking on this caregiving role is the main message in his book, “My Two Elaines.” Sharing his knowledge by writing the book has been Marty's late-in-life career.  “My Two Elaines” is more than an accounting of Marty’s struggles in caring for his wife. Activities are important for keeping someone living with Alzheimer's engaged and productive. However, as their disease progresses activities they once loved become impossible to manage. Activities offer important benefits like cognitive stimulation, the opportunity to connect with another person, relaxation and more. If you're caring for a loved one with Alzheimer's you may struggle to find activities that they are capable of doing without adding stress to your day. What options do we have to keep them busy and happy? Finding the right balance between what is good for them and what is good for you is an endless challenge. It's a challenge that is fraught with emotional struggles. Is it even worth bothering? Taking what they once loved and simplifying the task is a great start. Unfortunately, our being aware of what they can actually accomplish. Sometimes we have to take a step back and see if we're expecting more from them than they are capable of handling. Not only have  200+ trials looking for a cure for Alzheimer's failed, a cure in my life doesn't look likely. What if we have had one all along? According to Dementia Care Central the dementia-related conditions that can be helped by CBD include; Alzheimer’s disease, Vascular Dementia, Lewy bodies dementia, Parkinson’s, Frontotemporal dementia and Huntington’s. Additionally, according to researchers at California’s Salk Institute, their 2017 study has found evidence that cannabinoids such as CBD could help remove dementia from brain cells. At the same time, we're looking for a cure for Alzheimer's, we're ignoring a potential natural ally. At the very least it could be a preventative treatment. In the hope that we can avoid developing the disease, or at the very least reducing its effects, are we missing out?  Why are we afraid to investigate this option? In light of this idea, I spoke to Chela of AlzNotes about her experience with her Mom and her use of medicinal cannabis. Coupled with her personal experience, Chela has done more than her share of research on this topic. Given that I have a family history of Alzheimer's I thought it was important to learn more on this topic. Has There Been Research Into Cannabis &amp; Alzheimer's? Notably, there has been a lot of research on cannabis as medicine. A 2014 study published in the Journal of Alzheimer’s Disease showed that small amounts of THC can slow down the progression of Alzheimer’s in patients. Additionally, the right balance of CBD-THC can greatly reduce dementia induced anxiety or aggression. Another key point is that THC is known to be a powerful antioxidant with neuroprotective properties. However, the 2014 study reported that the compound directly affects Alzheimer’s pathology by decreasing amyloid beta levels. Obviously, this is a mind-blowing result. Additionally, as you'll hear, Cannabis has many other potential healing properties worth looking into. Natural cures come with far fewer side effects so they're worth researching. Could Cannabis be a Cure for Alzheimer’s?
