No matter what industry you are in, if you work with other people, there will be office gossip and drama. It’s inevitable, but there are ways to deal with it that leave people feeling safe, less stressed, and less impacted by the office gossip. Doc Jock joins the conversation because he’s dealing with this situation in his practice right now. Without going into the specifics, you’ll be able to hear and relate to the issue at hand and together, he and Sully come up with some ways to navigate the problem.  Listen for some ideas, like simple walks or lunches with team members or even personality tests to determine the best way to converse with your staff and the best way to group them in order to avoid the dreaded office gossip. It’s important to take a close look at metrics. They are important tools to use when making decisions. But the metrics you look at and the goals you set around them all depend on how you run your practice. There was a recent article penned by Mike Abernathy around KPIs for your practice. Mike joins the Millennial Dentist Podcast to talk about the points in the article and to share his experience in the field. There are some areas of agreement and disagreement and no matter where you are in your practice development, this episode has a lot of great takeaways. Ultimately, the goals you set are up to you. You can set lofty goals, but make sure they are attainable. But don’t sell yourself short and set mediocre goals when you can do so much more. This conversation with TBone is all about where Sully and the practice went wrong in keeping this associate and what they could have done better to prevent it. Failure isn’t a bad thing. 118. Sully Lost His First Associate

Welcome back to part 2 of a previous conversation with former associate Dr. Jackson. Dr. Jackson has recently taken the opportunity to shift from associateship to ownership and with that change comes a lot of decisions. In part 1, Dr. Jackson shared his journey, how he made decisions for his new practice so far, and he laid his fears all out on the table. In today’s installment of this conversation, we talk more about the team he’s inheriting with the purchase of an existing practice as well as leadership skills Dr. Jackson will need to learn on the way to being the best leader the team can have. Shifting from associateship to ownership is a huge decision, but one that many dentists make. We are confident that Dr. Jackson has been prepared for this new endeavor through his work with us and are thrilled for this new opportunity for him. 117. Associateship to Ownership Part 2 with Dr. Jackson

Dr. Jackson is a current associate who is about to take the leap to ownership! It’s very exciting for him and we’re really happy for him and this opportunity. Of course it is a blessing and a curse because we are now faced with replacing him, but with transparency early on, we knew this would happen one day when we took Dr. Jackson on as an associate. Our conversation today is all about this upcoming transition for Dr. Jackson. He is moving from associateship to ownership and it’s going to happen quickly, so in addition to him sharing the process he’s gone through, he also takes the time to ask questions, share his fears, and discuss the big decisions he’s faced with. In this episode Dr. Peyray, the Holeshot, and myself divulge the current state of our loan affairs. When you graduate its super easy to get bogged down with student loans. I think it can be one of the biggest hindrances to people’s early success. There is such a balance of living with the debt and having a strategy to pay it off.  We look at the different government options such as  IBR, REPAYE, and others. We talk about refinancing and some of the programs that do this like SoFi and DRB. However, we spend most of the time talking about whether it’s a smart idea to aggressively pay your loans down or not. The most discussed resource in this episode is something we mention very often. Best thing you can do is check out the White Coat Investor! At the end of the day we want to encourage our listeners to think about retirement vs. paying loans down early. Make sure you educate yourself on all the options. If you are one of those people who just wants to pay your loans down as soon as possible then make sure you have a plan on how your going to save after and be able to give a good reason why this is your strategy! [05:44] Ashley talks about challenges of running a practice that include juggling family and mom duties, leading a team of young females, and still wanting everyone to like her. [06:08] Erin has evolved over time. She does try to keep some separation with her team. [07:11] Erin always brings everything back to the patient. She also asks her team for their solutions when they come to her for problems. [08:54] Ashley knows where she wants to take her office. She struggles with investing in technology and taking on debt. [12:05] Sully delegates everything. He jumps in and figures it out. [13:27] Erin thinks dentists need to work on their mindset and empower their teams. Find each team member's strengths and empower them. [14:28] Break down what you need to do to get to the next step. [15:30] Erin is about to speak on stage, so she is going to leave the show. [16:30] Vinh's timeline is to buy into a practice in the next few years. Benefits of working with other dentist owners is that you aren't alone. He wants to have equity in multiple offices. [18:29] Sully's dad still practices. [21:38] Ashley can expand her offerings or bring other people on. She could calculate her annual patient value. Then focus on increasing that amount. [24:18] At Vinh's practice, they keep most everything in house. [26:07] They also offer payment plans or cash discounts. [28:30] Ashley was booked doing fillings all day. She is working on investing in her business. [32:41] The Cone beam is better for diagnosis. [33:50] Give people who pay up front a 10% discount. You can also do 3 payments over the timeline. [39:42] Ashley was losing on opportunity by pushing new patients weeks away. She has two full-time hygienists. Now she is struggling with giving them hours. [41:11] It's important to be honest with the team members and give them extra duties if they are flexible. [45:22] Eventually Ashley will build out her other rooms as she builds and expands her practice. [46:37] You have to have everything dialed in before you add people and rooms. 078 The Pros and Cons of Being an Associate Versus Being an Owner with Dr. Vinh, Dr. Erin Elliott, and Dr. Ashley Joves

Fundamentals help to move the practice along with and to assist in defining the next step in your business. This episode will focus on a topic we will have for our next seminar - case acceptance. We will talk about case acceptance and how this relates to your practice. We talk about the many challenges we face as a dentist in 2020, such as challenging insurance companies, the corporate environment, direct consumer dentistry, the challenge of finding qualified staff. Many dentists are seeing a slow degradation on their practice, are working more hours or not feeling as professionally satisfied as a result of many of these challenges in our office.  The core fundamentals have the biggest effect on your practice. The buy-in process is one of the most fundamental processes you can change to experience a significant improvement in your practice. We are simply not doing what it takes to diagnose better, then when we do diagnose, we don’t have a buy-in process for our patients. The buy-in process can be broken down into two simple steps:  How do I handle patients who don’t fit traditional dentistry? How do I make it easy for my patients to afford procedures and dentistry they want?  We also need to remember the three basics that people won’t have work done: time, fear, and money. How are you addressing these principals in your practice, regardless of what type of practice you own? Find out what elements you can put in place to enhance your patient experience and encourage patient buy-in with clinical diversity. How can you find out what types of clinical diversity your patients are asking for? Learn the steps involved in diagnosing what kinds of procedures and endo you can add to your practice to evolve to the next level you are looking for in your practice. Dr. Campbell has created an entire institute called the Advanced Prosthetics Institute in Prescott, Arizona to teach dentists on the importance of taking a biofunctional approach to dentistry. Holsinger, Peyray, and myself sat down with Dr. Campbell for about an hour to hear his journey from associate to owning a niche practice that deals with complex patients and care. Dr. Dichter is the resident prosthodontist at Spear Education. He brings with him 20 years of clinical, research, and teaching experience. He is also an instructor for the upcoming workshop  Restoring the Edentulous Arch. Sully and Dr. Dichter kick the show off with Dr. Dichter sharing his background growing up in Oregon, and how he almost become a ski instructor instead of a dentist. They also talk about how Dr. Dichter grew a practice from scratch before deciding to do a residency. We also learn about the chance phone call that ended up placing Dr. Dichter at Spears. He went to college at the University of Oregon. He had a family friend who was a dentist, and that inspired him to take the Dental Admission Test (DAT). [04:27] He had to choose between being a ski guide or a dentist. He chose being a dentist.  [07:22] He went to Oregon Health Sciences in Portland. After school, he worked with a family friend for a year, and then started a practice from scratch. [12:01] Some dental schools are designed to make people really good clinicians, and others are designed to prepare people for the next step. It would be helpful to know what you wanted to do before choosing a school. [13:31] When working in a small town everyone knows who you are and where you are. It's still easier to start a practice in a small town. [16:08] He then moved to Portland to practice. He was at a lecture and was approached to join a study club.  [18:39] The most impactful study club that Dr. Dichter was in also had the periodontist, orthodontist, and endodontist that he worked with as members. They would study past cases to know what they would want to do differently on new cases.  [23:37] He ran a practice in Portland for about 5 years, and then decided to do a residency. [24:11] He ended up going to UCLA.  [28:33] He almost started a practice in Orange County, but was invited to work at Spears.  [30:16] He still sees patients and then has workshops.  [31:32] He is looking for specific patients to fulfill certain dental needs. He does comprehensive restorative dentistry.  [33:44] The challenge is that everything is designed about a technique, but people need to learn the other options. And then need to know which system or technique would work the best. [40:44] Everything we do digitally is fundamentally the same as the analog world. It's a tool that could make you more efficient or more predictable. [45:36] You learn so much by doing your own lab work.  [49:01] Preserve the teeth, because it will preserve options for you.  [50:34] Find conservative options if you can. 089 Creating a Dental Career That You Love with Darin Dichter

This is our first non dental topic that we have had on the podcast! In fact, if you have a “High Deductible Health Plan” then no matter what you do, this episode is for you! One of the best tax advantaged accounts people can have is a a Health Savings Account or HSA, and if you have a high deductible health plan then you and your family are eligible to open up an HSA. The beauty, as we will discuss, is that the money goes in tax free, grows tax free, and if used on medical expenses comes out tax free! Thats triple tax free folks! Of the various non dental topics to consider this was the first one I wanted to tackle because for me personally its had some relevance of late. In the episode, I discuss my wife’s recent knee surgery and what it looked like for us to have a high deductible plan and the choices we had having opened up an HSA last year. Obviously student loans are getting out of hand and its probably the number 1 question students/new graduates have questions about. [14:06] 80% of getting anything done in life Is knowing why to do it. [14:53] To get your team to buy into the vision, you need to let them help create the vision. [16:09] When talking about transformation use emotion evoking words. [16:33] To create a team mission statement have everyone write down emotion evoking words that express their calling in dentistry. [25:08] The four cornerstones of building unstoppable team include shared vision. Begin your staff meeting by reading the vision and sharing. [27:22] It's also helpful to acknowledge people. [28:25] A set of operating principles that we call agreements. These provide certainty. Have the team decide how they are going to be together. [31:05] Communication is the third cornerstone. It needs to be open positive communication. [33:41] A reality check is a communication tool that is a series of questions that allows you to stop and reassess the situation no matter how upset you are. [35:57] The fourth piece is relationships. Having a shared vision, agreements, and communication leads to a positive team relationship. [37:55] Coaching is taking on someone else's commitment as if it was your own. Then interacting with them in such a way that they get the desired result. [43:26] If you want to find out what people value, ask questions. [45:48] Being on a cohesive team is what keeps quality people around. [48:08] It's not about making money now, it's about creating a plan so that you are free and money is abundant. [53:11] Sully has been in a funk lately. As the leaders go, so the team goes. [54:04] We need to be able to manage our emotional state no matter what is going on around us. The most profound thing Dr. Bass learned from Tony is state management. [55:15] You can change your focus by asking yourself a better question. This can change your thought process immediately. [58:14] A coach can ask you questions that drives you towards your vision. [58:37] The ultimate success formula. Number one know your outcome. [59:42] 500mg of niacin every day to lower your cholesterol and two tablespoons of Metamucil in water before each meal. [01:02:27] A decision is to cut away from something. If you have a behavior that isn't empowering cut away from it. [01:02:55] Master your mind, body, and relationships. [01:04:04] Create a lifetime plan and work backwards. [01:05:00] Create outcomes as opposed to goals. [01:05:19] To accelerate numbers in your practice, accelerate your hygiene re-care program. 015 Two Millennials That are Changing the Game!

After meeting up at the “Voices of Dentistry” in Nashville earlier this year, we knew we had to sit down with Danny Domingue. As far as we were concerned we hit it off and loved what this guy had to say about implants and being a badass young dentist. We were also a couple fireballs deep with the Dentists, Implants, and Worms crew so that might have had something to do with it being so memorable!  This interview reminds me more of a fireside chat then an interview! We dive into the pathway to placing implants for a new grad, importance of continuing education, the future of implant dentistry, and a bunch of other random interesting stuff. As the youngest diplomat ever for the American Board of Oral Implantoligists, Peyman and I were just trying to extract info! Danny is currently the President of the membership committee for the AAID, owner of a practice in Lousiana, and does a good bit of teaching on the side. They still grew, but it wasn't his best year. [03:22] The first step to moving forward is reflecting on the past. [03:39] T-Bone didn't set goals or plan out his 2018. He feels that this lack of planning showed. [03:55] He's actually a big believer in sitting down and writing out goals. This year he went to a seminar about planning out your goals for the year. [04:35] T-Bone was here in town for the seminar and decided to stop by the office for the podcast. [04:57] It's not only important to write out your goals. It's important to share your goals and have accountability. [05:14] Last week Sully and his office took half a day to recap what has happened in the past year of 2018. [05:50] It's important for the owner of the business to recap before he recaps with the team. [07:53] It's a good idea to map out what you are going to say before you discuss it with your team. [09:14] For T-Bone, 2018 was a non clarity year. If you don't have clarity you're going to be in trouble. [10:33] T-Bone makes the same resolution every year. That is to eat healthy and lose weight. [10:58] He feels that the best time to plan for the following year is in September and October, so that you really have time to set the stage. [14:10] To get further faster, you have to focus on your patients. You have to focus on your team. You have to focus on your clinical skills. [14:42] With patients you have to address their time, their fears, and their money. [15:12] Do more in one visit, make your dentistry last longer, and make appointments more convenient. [15:42] T-Bone's office is going to go to a 7:00 to 7:00 schedule. [18:26] Do something different to be there for the patients. [19:13] The easiest way to address fear is to offer sedation in your practice. Nitrous Oxide, oral sedation, or more advanced sedation. [22:47] Good old-fashioned TLC also helps address fear. [23:05] Money boils down to price vs affordability. [25:02] Affordability is about offering payment plans versus the price of the procedures. [25:48] Step two is addressing your team and how you lead your team. [26:17] Teach your team to learn how to replace themselves. Teach your team to know the score. Teach your team to own the space. [26:38] Replacing yourself is about personal growth. If you train a replacement, you are able to move up. [32:19] When you understand what's going on in your team members lives, then you can formulate a plan with those person. [33:18] Just listen and avoid the advice monster. [34:03] Ask questions to get them to get to your advice. [37:56] Number two is about knowing the score. Be specific with your team members about their role and how you want them to grow. [41:28] Ease people into new roles. [45:06] Owning your space is about knowing everything that happens in your operatory. [46:46] People want ownership and leadership. Make certain jobs certain people's responsibilities. [47:30] Teach people to replace themselves, so they can advance. Know the score and specific things about your part of the practice. Teach people to own their space. [50:31] Superstars get different treatment but everyone gets treated fairly. [50:49] Clinical diversity. To get further faster focus on three areas of your professional life. [51:31] We have to make a progression beyond dental school dentistry. [52:02] The top five services are perio, hygiene, restorations, extractions, and endo. A practice is moved to a level two practice by implementing the bottom 5 procedures which are socket grafting, implants, ortho, sleep, and sedation. [52:54] Implement the bottom five procedures at a high level. [54:23] T-Bones goal for 2020 is to replace himself and only do the procedures he wants to do. He created the Crabtree Group about 12 years ago. Cameron joined 10 years ago. They both had a background in sales and marketing. [23:16] Two big challenges are getting the phone to ring and what you do after the phone rings. [24:08] Prepare the patient on the intake call. Qualify the patient. [25:50] Ask open ended questions and bring the patient along on the journey. [29:40] People in healthcare want to help. Instead, take control of the call. Answer the question with a question. [31:48] Sully and Sean do a mock phone call walk through. [34:06] It's helpful to record the calls or have them transcribed. [41:31] It's too much for the doctor to have to sell every single case. [16:59] Sean started about 22 years ago. He worked in a telecommunications business. He had a client who was a consultant. He wanted an opportunity to make an impact and started working with this consultant. [19:45] Sean discovered that people in healthcare didn't understand value creation or sales. He created the Crabtree Group about 12 years ago. Cameron joined 10 years ago. They both had a background in sales and marketing. [23:16] Two big challenges are getting the phone to ring and what you do after the phone rings. [24:08] Prepare the patient on the intake call. Qualify the patient. [25:50] Ask open ended questions and bring the patient along on the journey. [29:40] People in healthcare want to help. Instead, take control of the call. Answer the question with a question.
