In this episode, Therese Markow and Dr. Leon Barron discuss what is in our wastewater is, both before and after it goes through the treatment facilities. Wastewater treatment can vary among countries and even within a country, depending upon local population density and regulations.  Profiling what enters a water treatment facility reveals a lot about the health and pharmaceutical consumption of a given population. For example, cocaine use is higher in Europe than in the USA where methamphetamine is more commonly found at high levels in pretreatment wastewater. Even with treatment, not everything is removed from the water that we generally assume to be safe. For example, Dr. Barron’s recent research reveals that cocaine and other psychotropic drugs have been found in the rivers in “pristine” environments and that these drugs show up in the invertebrates and the fish that eat them.  While low levels are currently assumed to be safe for humans, longer term and more in depth studies on these levels for humans and fetal brain development are critical. A holistic approach to his clinical practice has led to an interest in medical cannabis as an alternative therapy for patients who have not found conventional therapies to be effective.Dr Barron is an executive committee member of The Medical Cannabis Clinicians Society which provides high quality, balanced education and expert support to inform clinicians about the current state of evidence with regard to the efficacy of cannabis and its side effects. He is also the founder of The Primary Care Cannabis Network which offers a platform for GPs with an interest in medical cannabis. He continues to undertake regular NHS work and is a visiting tutor for undergraduate medicine at University College LondonEpisode SummaryDr Barron became interested in Medical Cannabis two years ago after hearing about cases in the media. At the same time, his relative in Israel was responding well to Medical Cannabis as a treatment which further sparked his interest.Leon explored the topic further by researching and finding scientific journals on PubMed where he came across the work of the early cannabis scientists.He identified ways in which medical cannabis could change clinical practice within the UK and then went on to meet a number of cannabis experts and specialists within the UK such as Mike Barnes who fuelled his interest.As of November 1st 2018, cannabis based drugs for medical use was made legal within the UK as prescribed by specialist doctors, by prescription only. GP’s are not able to issue prescriptions.Leon encounters 2-3 patients per week in his general practice who ask about medical cannabis. He has also noticed a great interest from patients in CBD products.In the UK, the general view of practitioners is that a patient should have tried conventional therapies before being offered a cannabis based medication.For somebody who is experiencing pain, the first referral a patient would get (on the NHS) would be to a pain specialist, who are unlikely to prescribe cannabis based medication due to lack of knowledge. Another option is being seen privately, where the patient will have to fund the cost of the medication themselves, making this a dead end option for many. Another option would be to enrol in the 2021 trial which is being run by Drug Science.
