In this episode, Dr. Mikhail Regelman and I continue a conversation about how we manage issues of incontinence and impotence after man has had his prostate removed for prostate cancer. In our last episode we focused on postoperative incontinence. In this episode we discuss the protocol called the SIMS (Surgical Impotence Management Strategy) protocol for helping men restore sexual function after their operation. In this episode we cover a broad range of topics including: what is a nerve sparing approach to a prostatectomy and why it matters for sexual function after the operation, what specifically is our SIMS protocol, how to use vacuum erection devices or injection treatments for sexual activity after an operation, what to expect as far as recovery of sexual function after the operation and Dr. Regelman tells us why we should think of our penis as a sports car.  Here’s my conversation with Dr. Regelman.

Prostate Cancer-SIMS: Impotence with Dr. Mikhail Regelman ep 27

For this episode I interview one of my young partners Dr. Mikael Regelman about care for patients after having their prostates removed for prostate cancer. Doctor Regelman and I sat down together after clinic one day to discuss what we call our SIMS protocol. The SIMS protocol is a preoperative class combined with postoperative clinic visits for men who are having their prostates removed for prostate cancer. We discuss the anatomy relationship of the bladder, prostate, and urinary sphincter and how it changes after the prostate is taken out. We also discuss our expectations for men in terms of the recovery of urinary control as well as strategies such as Kegel exercises and biofeedback to encourage and restore urinary continence.
