Anthony Llabres and I discussed Testosterone back in Episode 27, “Why Is Your Manhood Slowly Seeping Away?”   As a sort of follow-up to that conversation, and as part three of our Hormone Replacement Therapy series, we talk about BHRT for men and touch base on Andropause, AKA “Manopause.”   We also answer some of the questions you all wrote in after listening to Part 1 and Part 2 of the Hormone Replacement Therapy series!   What is Andropause?    It’s a usually slow but steady reduction of the production of testosterone and dihydrotestosterone, which are the main androgens or male hormones. The ten most common causes of death in males are heart disease, cancer, accidents (or unintentional injury), stroke, lower respiratory diseases, diabetes, fluid pneumonia, suicide, kidney disease, and Alzheimer’s... and when you look at those ten causes it’s pretty clear that andropause can play a role in nearly all of them if not all of them.    The symptoms of Andropause (it’s a short list, but so real):    Fatigue Depression Weight gain Bone and muscle loss Foggy thinking Difficulty concentrating Anxiety Irritability    Factors that Drive Andropause - The Big 3:    Insulin Resistance Cortisol/Stress connection Estrogen Dominance    The Liver’s Role in Healing and Reducing Symptoms of Andropause:   Liver health is key in healing or reducing the symptoms of Andropause. Conversely, abusing the liver can worsen Andropause.   If your liver isn’t working properly, it can’t metabolize excess estrogen, it can’t metabolize sugar properly – leading to insulin resistance. All of the big 3 contributing to Andropause – insulin resistance, estrogen dominance, and cortisol problems – can impact the liver. Ep. 46 - Hormone Replacement Therapy: Andropause (AKA Manopause) (Part 3) - with Anthony Llabres

Between the “expert” at Whole Foods, your hairdresser, and your most savvy friend, it’s hard to know who or what to believe when it comes to hormone replacement therapy (HRT). So for part two of our HRT series, we’re going to dispel the myths and highlight the truth around HRT.   If you haven’t listened to Part One, you can go back to learn more about what HRT is, its history, and our two expert guests: Marie Hoäg, an independent clinical hormone medicine consultant, and Anthony Llabres, a Certified Functional Diagnostic Nutrition Practitioner (FDN-P). So let’s get into some of the biggest HRT myths:    There’s only one kind of HRT. In the last episode, we covered the four generations of HRT. On top of that, all forms of HRT are not created equal. Therapies using low, static doses are just not as effective, and Bioidentical Hormone Replacement Therapy actually mimics a woman’s hormones in her reproductive prime.     HRT causes cancer, blood clots, and cardiovascular disease. There’s a lot of misinformation about estrogen, testosterone, and cancer.   Estrogen alone has never been shown to increase the risk of breast cancer. The combination of some Estrogens and Progestins has been shown in some studies to slightly increase the incidence, but other studies have not confirmed that. Progesterone in conjunction with Estrogen, however, is required if the patient still has a uterus as Estrogen alone increases the risk of uterine cancer. Anthony points to the work of Abraham Morgentaler, MD, to disprove the myths around testosterone and prostate cancer:  "Destroying the Myth About Testosterone Replacement and Prostate Cancer".         Saliva is the best way to test for hormones. This just isn’t true for the more advanced HRT systems. There are actually physical, mental, and emotional symptoms for hormone deficiency, but the most reliable way is blood serum testing. Marie’s view is that if there is not enough estrogen circulating in the blood, then a woman just doesn't have enough.       Any doctor can prescribe HRT. Technically any doctor can prescribe HRT, but they don’t teach this in medical school and, without the proper training, they may not understand how to test and administer hormones correctly.     Only women in menopause need HRT. Menopause just means that your period has stopped for 12 months; for whatever reason, your ovaries have shut down and you’re no longer producing abundant amounts of estrogen. However, Marie is a big believer in menopause prevention – “Women never have to go into menopause, as far as I’m concerned. If you maintain a healthy menstrual cycle, then you never have to go into menopause. And that can be done with abundant levels of estrogen.”       Restoring menstrual cycles with HRT is bad. As we addressed previously, no women needs to be in menopause – and if we can restore their natural menstrual cycle, they’re better off.       HRT should only be done in low doses and only for the shortest time possible. In reality, the best results we’ve seen come from advanced rhythmic dosing that simulate physiologic dosing levels. You need the right hormones in the right amounts in the right manner, and what’s “right” isn’t the same for every individual.     Bioidentical means natural. Bioidentical hormones are NOT natural – the only way to get a natural bioidentical hormone is to  drink horse urine or eat the soy plant,  as reported by Lauren Streicher, MD  – but it does have the same molecular makeup as the human body’s.     HRT will not affect weight issues. Women need estrogen in abundant amounts to maintain or recover their figure; estrogen dictates the shoulder-waist-hip ratio. Ep. 44 - Hormone Replacement Therapy: Myths & Truths (Part 2) - with Marie Hoag & Anthony Llabres

I’ll be joined by two guests, Marie Hoäg and Anthony Llabres, to cover what BHRT is and everything you might want to know about it. Below is a breakdown of each generation, offered by Marie:    1st Generation - Oral pills, synthetic, lowest dose possible for the shortest amount of time, static dosing, no real education/training, and stops working in weeks.     2nd Generation - Compounded, bioidentical (this is the biggest difference between generation one and two), lowest dose possible for the shortest amount of time, static doses, some education (but not a lot), no clinical application training, designed to treat symptoms of hormone deficiencies (but not fix them), and stops working in months.     3rd Generation - Compounded, bioidentical, derived from a plant but still synthesized, medium dose, rhythmically administered (mimicking the natural physiology of each hormone), minimal theory education, no clinical application training. It’s better than 1st and 2nd generation in terms of clinical outcomes, but still only about 20% effective.     4th Generation - Compounded, bioidentical, physiological dosing (doses mimic a woman’s hormones in her reproductive prime), clinically effective dosing schedule, rhythmically administered, clinical algorithm, standardized customizable system, eliminates hormone deficiencies (not just the symptoms), 80-90% clinically effective, and sustainable “Hormone Sweet Spot”™.    A big part of the 4th generation’s success is because the size and schedule of the doses more closely mimic natural hormone cycles. You can see in  Moxie’s chart that hormone levels fluctuate throughout the month, and rhythmically administered doses follow that same pattern. When you have a flat dose, it just creates an estrogen dominance effect and women can’t detoxify it.   Anthony tells us that Dr. Jonathan Wright of the Tahoma Clinic probably wrote the first prescription for BHRT back in 1982. And because men are conditioned to “suck it up” and may not be as open about sharing their symptoms, particularly these symptoms, it often goes unaddressed.   But now we have coaches like Anthony out there educating men on this topic, and showing them that they don’t have to live with these symptoms either. And because men are conditioned to “suck it up” and may not be as open about sharing their symptoms, particularly these symptoms, it often goes unaddressed. But now we have coaches like Anthony out there educating men on this topic, and showing them that they don’t have to live with these symptoms either.
